the CRB will return forms not completed in black.
Section A - all questions to be
answered including 12 (N I Number)
Section B - do not alter - the description 'Leader/Helper' covers
all Scouting adults.
Section C - an applicant must enter
all former birth/maiden surnames and forenames where applicable, especially if a
cross is entered in the 'Mrs' or
'Ms' field or a title is entered in the 'other' field.
Section D - you must ensure that ALL
previous addresses for the past 5 years (ie a full 60 months) are
entered
correctly, especially the 'From' and
'To' dates which must abut leaving no gaps of apparent homelessness.
Section E - leave blank - there is
no need to complete
Section F - leave blank -- there is
no need to complete
Section G - leave blank
Section H sign
once only at 68 do not sign 69.
Section X - the minimum requirement is at least one Class 1 identity document ie a passport or a driving licence (paper or photo) or a birth certificate that was issued within 12 months of the date of birth. If none of these documents is available the DC should be contacted for advice.
(If no class 1 is available then 5 class 2 documents are required
instead, but they have to be listed on a form which is held by the DC. These can
be Birth Cert (not issued within a year of birth), current TV license,
Bank statement, utility bills etc.)
- make sure you complete all boxes
appropriate to the document being checked
- ensure that the date of birth you
enter agrees with the date in Section A line 10/11
- print the first name & surname of the checker in line 16
Section Y - leave blank - do NOT
sign (this is for HQ to complete)
Section Z - leave blank
Finally contact the Local Scout Fellowship Chairman to sign that he or she has seen and checked the appropriate documents. Have these ready for his/her visit.
